- Job Details
- Bachelor or Master Degree in Occupational Therapy
- Current state Occupational Therapy certification/license
- Minimum 1 year experience
- Job Requirements
- Current BCLS (AHA) certificate
- National Board for Certification Occupational Therapy
certification, AOTA and OTAC preferred
- Additional Details
- 2 COVID-19 vaccines and booster required or declination completed
- Proof of 2 Vaccines or Positive Titer showing immunity
- Annual Respirator fit test
- Flu Vaccine or declination required October 1-May 31
- Vaccination/titer showing immunity
- Driver's license, State ID, or Passport
- MUST RECEIVE PROOF OF POSITIVE SKIN TEST AND PROOF OF CHEST XRAY
- National Criminal Background Check within one year of start date
